I did a quick test... A course not been backed up for a month as I had "Skip courses not modified since previous backup" ticked and it hadn't been touched. I enrolled a student on the course, no further changes and when the next automated backup ran the course was backed up. So it seems firstly that an enrollment is enough to trigger this and there must be other clauses that the automated process looks for when this option is selected.
