Our practices are the same as yours, and we too are experiencing this.
I used your report (thank you) and sure enough the course that I've been unable to delete for months now showed up. Thankfully, it was one of only 3.

Also, your answer to Ken is our answer. We used to allow reset, but no longer. All courses are kept for 3 years.
