I inherited this project from another developer, so I had to do some digging.
After double checking, nothing should be getting cleaned up while this process is running. I don't think the way it's done is standard so I'll be looking at that, but that does not seem to be the issue.
The backup folder only contains the individual log files, no directories. The log files don't contain any useful information (only the status changes). This makes me think that after it fails, it does get cleaned up. I can try and find out where that happens and then disable that so we can perhaps dig into that further, but we've already grabbed the backups of several failed restores, and have seen nothing out of the ordinary within them.
We have set up a separate instance of Moodle 3.11.18, without data, and attempted to restore a backup into that instance. Importing a question bank succeeds just fine, but restoring a course has the exact same issue we're seeing on our main instance. I think this really points us to a data issue and rules out our custom plugin.
All assignment modules are converted to the new assign, and there is nothing left in the database relating to the old assignment modules, so we're good there.
