Suggest making a query of mdl_files for a known PDF file name to see if it does exist there ... and the context/module ID number (module id would be in course).
Doesn't exist there, won't be in files.xml of backup of course.
How good a shape is moodledata/filedir/ ?
Was site config changed - location of moodledata or was site migrated?
Suggest also installing command line moosh. It has a command to check mdl_files table with actual files in moodledata/filedir/ ... you could have 'orphaned' references in table but not physcially in moodledata/filedir/ or vice versa ... files there but not DB reference.
