I appreciate your thorough reply... I was neck-deep into this when I read your ideas, and if I had access to 3.5.1 and could run the Admin Health tool, I would have... believe me. However, in a pinch, here's what worked for me.
Rename the backup file (.mbz) to zip and extract.
Open questions.xml and rename every to a unique value. Select all the files (while in the extracted dir) and re-zip. Rename to .mbz and upload.
Wow. That'll be fun if there are multiple users with this issue...
